What should I do if I am asked to make a contract form to hire me by the construction employer that wants to hire me?

What you need to do is prepare a letter that says what you're agreeing to do in exchange for $X per hour for Y period of time.  If you're smart, you'll have an attorney prepare a simple contract that binds you and the construction employer to live up to the mutual promises of what you've both agreed to do.
